Our goal is simple:

To better understand how athletes move, perform, and develop so we can provide meaningful guidance for long-term growth.

This agreement outlines participation expectations and permissions related to athletic evaluations, training, camps, programming, video analysis, and future development services provided by Hilt Performance LLC, powered by Get Grit Ready Inc.

PARTICIPATION ACKNOWLEDGEMENT

I understand that participation in Hilt Performance LLC activities may include:

• Athletic evaluations and combines

• Sprinting, agility, and jumping activities

• Strength and bodyweight exercises

• Balance and movement assessments

• Sports performance training

• Camps, clinics, classes, and memberships

• Individual or group coaching

• Video-based movement analysis

• Digital and technology-supported development experiences

I understand these experiences are designed to evaluate and support athlete development and are not guarantees of athletic performance, injury prevention, or future outcomes.

By signing below, I voluntarily consent my child to participation.

ASSUMPTION OF RISK

I understand that participation in athletic activities involves inherent physical risks, including but not limited to muscle soreness, strains, falls, physical injury, aggravation of existing conditions, or other risks associated with athletic movement and training.

I confirm that the participant is physically able to participate and that any known medical concerns, injuries, or limitations have been disclosed during registration.

I voluntarily assume responsibility for participation.

MEDICAL AUTHORIZATION

In the event of an illness or injury requiring emergency medical attention, and if a parent/guardian cannot be reached, I authorize Hilt Performance LLC representatives to seek reasonable emergency medical care for the participant if necessary.

I understand that any medical expenses incurred remain my responsibility.

VIDEO & PERFORMANCE CONSENT

I understand that Hilt Performance LLC uses video, photography, movement observations, and performance tracking as part of the athlete development process.

I consent to the recording and storage of athlete performance information for purposes including:

• Movement evaluation

• Athlete reports

• Performance tracking

• Coaching feedback

• Development planning

• Internal coaching education and quality control

I understand these materials may be stored securely as part of the athlete’s development profile.

DEVELOPMENT DATA & TECHNOLOGY CONSENT

I understand that Hilt Performance LLC, powered by Get Grit Ready Inc., uses athlete performance information to improve athlete development systems.

I consent to the collection and secure storage of:

• Testing metrics

• Performance observations

• Movement videos

• Progress tracking

• Athlete reports and recommendations

I understand anonymized data may be used internally to:

• Improve coaching systems

• Improve athlete development programming

• Improve reporting systems

• Support future technology development

• Develop AI-assisted coaching recommendations and athlete insights

I understand personal information will not be sold or publicly shared.

COMMUNICATION CONSENT

I agree to receive communication via email related to:

• Scheduling and reminders

• Athlete reports

• Development recommendations

• Training opportunities

• Camps, programs, and memberships

• Educational content related to athlete development

RELEASE OF LIABILITY

To the fullest extent permitted under New York law, I release and hold harmless Hilt Performance LLC, Get Grit Ready Inc., and their employees, coaches, volunteers, contractors, representatives, and affiliates from claims arising from participation in athletic activities, except in cases of gross negligence or intentional misconduct.

Optional — Marketing & Educational Content Permission

I authorize Hilt Performance LLC to use photographs, video clips, or non-sensitive content featuring the participant for:

• Website content

• Social media content

• Educational materials

• Marketing and promotional materials

I understand names and sensitive information will not be shared without additional permission.
